Camp White Pine Pre-Registration Form

If you are interested in registering your child for Camp White Pine, you can:

1. Send a deposit of $100 (per camper/per session) to brookeaverill@hotmail.com 

2. Fill out this form with the following information:

~What session you have chosen
~Your child’s name & birth date          
~Your home address

3. Add a Camp White Pine t-shirt to your order for $25 and receive it on the first day of camp! (Please include quantity and size in your order.)

SIZES: Youth S, M & Adult S

An email will be sent to confirm your deposit and information have been received.
